Does the application need to be in the telephone customers name or head of households name?

Telephone Discount: The application may be submitted by the head of household, and the address listed on the application must match the address on the telephone bill (telephone customer). If another member of the household is receiving benefits from a qualified program, they should be identified in Section 1 of the application. The certification form MUST be completed by the telephone customer receiving the discount.
