I am an assisted living manager and wish to apply for the Lifeline discounts for some of my residents. When I have the residents complete the application, do I need to mail them individually to the Lifeline Texas program? |

Once the assisted living manager receives the completed application and certification form with all required documentation, they may send all of the applications and required documentation to:
