I have received a letter stating my Name, Address, Date of Birth or SSN does not match what is currently on file.
If you have received a letter stating that your Name, Address, Date of Birth or SSN does not match what is currently on file you will need to reapply for your Lifeline Benefit. Depending on the type of service you have you will need to either contact your provider or reapply directly with the Texas Lifeline Administrator as follows:

• For Prepaid phone service you should contact the service provider. That provider will instruct you on how you can submit the information to reapply.

• For all other providers, the telephone company cannot process your form. You can apply online or print forms at https://www.texaslifeline.org/ or call by calling toll free 1-866-454-8387.

A prepaid mobile device is a phone for which credit is purchased in advance of service use. These types of phones are commonly referred to as pay-as-you-go, pay-as-you-talk, pay and go, go-phone or prepay.

All "other providers" include traditional land line or contracted mobiles service.
