I received a letter that says my application has been denied.
If your application is denied but you have additional information that could make you eligible for the discount program or if the original document you provided was missing information, you can send that information into Texas Lifeline. Some examples are:

      • If you were missing a signature on your application, you will need to re-submit your application with the
        appropriate signatures.

      • If you were missing documentation, you should send in the documentation showing eligibility for a qualified
        program or income documentation.

      • If you were missing a Lifeline Certification and/or Household Worksheet (IEH) form you will need to submit the
        completed form(s) before you can be approved.

The supporting documentation that is being asked for in the rejection letter should be sent to the address supplied in the letter. You should include a copy of the rejection letter in your response:
