What is the Lifeline Household Worksheet
The FCC allows for more than one lifeline discount per unique address if the matching lifeline participants at that address certify that they are separate households. In other words, they do not share any expenses. A Lifeline Household Worksheet is a form that is used to certify that you are a separate household. The FCC requires that only one Lifeline discount is allowed per household. The Lifeline Household Worksheet explains this situation and allows you to complete a questionnaire to determine if your situation qualifies as a single household or as multiple households.
